The Heartland Alliance has commenced its transition process in Cross River State, with plans to hand over patients and relevant structures under its care to the Excellence Community Education Welfare Scheme (ECEWS). The organisation disclosed this during a briefing with the Cross River State Agency for the Control of AIDS (CR-SACA), where its State Lead, Ajah Igiriga, briefed the Director-General, Dr. Charles Iwara, on the transition process. Igiriga said Heartland Alliance would ensure that all activities and services it had been providing are properly captured in relevant systems and incorporated into the State's response as the transition progresses. He added that all patients currently under the care of Heartland Alliance would be handed over to ECEWS to ensure continuity of care. Responding, the Director-General of CR-SACA, Dr. Charles Iwara, stressed that continuity of care remains the most critical consideration in the transition process. Dr. Iwara urged that every patient be properly transferred to ECEWS, while relevant structures are adequately captured to prevent gaps in care and ensure a smooth transition. He expressed confidence in the capacity of ECEWS to take over and sustain the necessary care and support services. Also speaking, Dr. Joseph Umana of ECEWS assured that the patients would be in good hands, adding that the organisation would do its best to ensure seamless continuity of care throughout the transition. The engagement provided an opportunity for Heartland Alliance, CR-SACA and ECEWS to coordinate the transition and ensure that patients and structures are properly handed over without disruption to care.
